
Patient Confidentiality Policy
All employees and agents
working for or in relation to Valley Women's Clinic, P.A. are bound to
maintain medical records on individuals in accordance with the Laws of the
State of Texas and the Privacy Act of 1974, 5 U.S.C. § 552A, As Amended.
Information provided by patients is to be utilized for treatment purposes
only. All patient medical records are maintained in a private and
confidential manner so as not to be accessed by any unauthorized individual.
Conditions of disclosure,
per the Privacy Act of 1974, medical records shall not be disclosed by any
means of communication to any person, or to another agency, except pursuant to
a written request by, or with the prior written consent of the individual to
whom the record pertains.
Patient information will not
be released until the patient has signed the Valley Women’s Clinic Release of
Confidential Information Form.
Under certain circumstances,
information may be released to designated agencies, as defined by the Privacy
